AFC Program Nurse The Adult Family Care program is funded through the Commonwealth of Massachusetts MassHealth. Families who qualify may receive approximately $9,000 (Level I) or $18,000 (Level II) for providing care for their adult child with developmental disabilities. JOB SUMMARY: The AFC Program nurse works closely with the program Care Manager and is responsible for initial and ongoing assessment of both the setting and care provider. The Nurse will assist in assessment and development of the Plan of Care in conjunction with the Care Manager. Review of the Plan of Care will take place through bi-monthly on-site visits with the family. Documentations necessary include completion of progress notes, review of caregiver logs, review and documentation of fire/safety procedures, and annual review of the home setting. Additionally, communication to GWArc's financial office will insure timely payment to both the caregiver and GWArc. Under the supervision of GWArc's CEO, who wrote the DMA application and holds the regulations and documents needed, program start up will progress from identification of qualified families already served by GWArc to organizing documents, team meetings with the AFC Program Care Manager and on site visits with families qualified to receive services. Further opportunity to outreach to elderly populations exists in the community and will help to grow this program. QUALIFICATIONS: GWArc seeks a skilled and organized candidate who has experience with populations with disabilities and/or the elderly. S/he must be a self starter who can work independently and is aware of timelines and the DMA regulations. Some flexibility in hours is required as on site home visits may need to be made later in the afternoon or evenings. No weekend hours are anticipated. The majority of families to be served will likely be in the greater Waltham area and mileage reimbursement is provided. JOB SUMMARY: To assist consumers to participate and benefit from a Work Training Program through the implementation of individualized program plans, interventions and work training routines at GWArc's Woodland Road site. Manage a caseload and develop and teach consumers as part of an Interdisiplinary Team of clinicians, health care staff and program staff. Consumers participate in BIS mailing employment and functional curriculum.
Work Schedule: 40 hour work week; Monday - Friday
QUALIFICATIONS: B.S. in Psychology, Special Education or related field with 1 year of experience. In lieu of a college degree, a High School diploma or GED with 1 plus years paid work experience in an instructor/care management role with a similar popluation.
Functional capability to lift, transfer, pivot and bear consumers' weight in a safe manner. Ability to implement multiple Behavior Modification Plans/ individualized interventions/ programs in a variety of settings/ environments. Commitment to community inclusion and upholding the human rights and dignity of people with disabilities, flexibility and responsive to a variety of requests. Ability and knowledge in signing, non-violent restraint, CPR/ First Aid. Good time management, organizational and communication skills. Knowledge of sub-contract work a plus.
